If your appointment is with Dr. Flo Barber: Hancock Holistic Clinic

If you have appointments with both, ONLY do the forms for Dr. Hancock.

If you are bringing a child under 18 years of age, or a Dependent age 18 or
older, to see either Dr. Hancock or Flo Barber-Hancock, please use this
Dependent History Form
